
Brookville Home Care Joins Medicare GUIDE Program Network
Brookville Home Care is proud to announce our partnership with Lizzy Care as an approved provider for the Medicare GUIDE Program, an innovative Medicare initiative created to support individuals living with dementia and the families who care for them.
The GUIDE Model, which stands for Guiding an Improved Dementia Experience, was developed to improve dementia care by offering families easier access to support services, care coordination, education, and respite resources.
For many families, navigating dementia care can feel emotionally and physically exhausting. Caregivers are often balancing medical appointments, safety concerns, changing behaviors, and the daily responsibilities that come with supporting a loved one experiencing memory loss. Through the GUIDE Program, families can receive additional support designed to ease that burden and improve quality of life.
Eligible participants may receive services such as:
- Personalized dementia care coordination
- Caregiver education and support
- Home safety assessments
- Respite care services for qualifying individuals
- 24/7 access to assistance and guidance
- Ongoing dementia-focused resources

Individuals may qualify for the GUIDE Program if they:
- Have a diagnosis of Alzheimer’s disease or another dementia-related condition
- Are enrolled in Medicare Parts A & B
- Live at home or within the community
- Are not enrolled in hospice care
- Are not residing in a long-term nursing facility
